Financial/Insurance Information

At Southwest Oral & Maxillofacial Surgery, P.A. we make every effort to provide you with the finest surgical care at the most reasonable cost. We accept Visa, MasterCard, American Express and Discover. Our office does not accept “Care Credit” and we cannot accept Medicaid.

Our fees for consultations start at $55, which is due at the time of service. Our fees for any necessary panoramic radiographs taken in our office start at $55, which is also due at the time of service. There are usually limitations on these services with all insurance companies. We will be happy to file a claim with your insurance company for these services in order to get you reimbursement from them to you for any coverage they may have.

We are preferred providers with several insurance companies, both medical and dental. Our office staff deals with these insurance companies and the many different dental and medical plans they may offer. Some companies combine dental and medical coverage. Many companies change policies and guidelines frequently and many insurance companies will not give out fees until after the treatment is completed. Frequently, a predetermination or pre authorization in writing is required prior to treatment. Dealing with insurance coverage and benefits can be difficult and time consuming. At times, it is almost impossible to accurately estimate our patient’s insurance co-payment prior to their treatment. We will make every effort to estimate your percentage of coinsurance with insurance coverage for your surgical procedure. Your estimated portion of fees for any surgical procedure is due at the time of service. We will file the remaining claim balance with your insurance company for you and will work to maximize your insurance reimbursement for any covered procedures.

HMO patients – If a patient comes to us with a problem that they expect to be covered by medical insurance, (biopsies, tumors, TMJ, infections, Jaw deformities) they must have a referral from their primary care physician. A referral from a dentist is not adequate for medical insurance coverage. Obtaining a medical referral is the patient’s responsibility. We cannot obtain the referral for you, and the referral cannot be obtained retroactively. If you do not have a required referral, we will be happy to see you on a cash basis, but your medical insurance company will not pay for your treatment.

MEDICARE Patients – Medicare pays us directly for your care. You will be responsible for any deductibles or co-insurance, if applicable, after Medicare has finalized your claim. Dental procedures (extractions, implants) and dental-related treatment are NOT covered by Medicare. If Medicare denies your procedure, you are responsible for the charges.
